The Glacier's Shoulder is a Nightmare Unique greatsword with no boss attached: it drops anywhere in Hell, most often from H10 up (rarer below that home difficulty, never above it), and nowhere else. Every copy is item level 110, and there 0.2% of what falls comes up Unique. It is built on Sovereign Nightglacier Greatsword — step 6 of the Nightglacier Greatsword line, item level 80 and up.
It always rolls the same numbers: +192 Power, −8% Attack Speed, +5% Melee Lifesteal, +146 Power, +-8% Attack Speed, +9% Frost Damage, +15% Power, +42% Melee Damage, +42% Frost Damage and +10% Melee Lifesteal.
One Unique drop in 40 is a Mythical copy: the same item carrying 15–25% Mythical Energy that boosts every stat on it and can never be changed; the 15% floor is 10 times as common as the perfect 25%.
On top of that it carries The Glacier's Shoulder — Heavy Cleave sweeps an 80° wider arc, shoves 1.5 units harder and applies Chill, but winds up 20% slower.
It leans into Chill.
Wearing it hands you Heavy Cleave and Groundbreaker.
“It does not swing. It moves, and things are elsewhere.”
